Overview
The CPSA Health Care Plan begins where your provincial plan ends.
Your provincial plan covers basic benefits such as hospital ward
accommodation, physician fees and any drugs or blood products
you may need while in the hospital. The CPSA Health Care Plan
takes over from there.
You have two options from which to choose: Basic or Deluxe.
Preferred Vision Services
You can receive discounts on prescription eyewear products when
you purchase the Health Care Plan.
Note: You, your spouse and dependent children must be in good
health to obtain this insurance coverage. Additional medical information
may be requested. Coverage may be declined or modified.
The plan covers expenses incurred in Canada only.
Eligibility
You can apply for coverage under this plan if you are a CPSA member
in good standing. You can cover your spouse and eligible dependent
children as well, provided that you:
- ordinarily reside in Canada and are covered under your Provincial
Health Insurance Plan;
- work at least 25 hours per week; and
- are under age 65.
Spouse means an individual under age 65 with whom you have lived
for at least one year, and who is publicly represented as your
spouse.
Dependent Child(ren) are your unmarried, dependent child(ren)
still living with you and under the age of 21, or 25 if in school
full-time.
Note: Your spouse and dependent child(ren) must reside
in Canada and be covered under a Provincial Health Insurance Plan.

Note: The Basic and Deluxe Health Plans do not cover fertility
drugs, smoking cessation, obesity products or other non-prescription,
over-the-counter drugs.
Termination of Coverage
Your coverage under this plan will end on:
- the date the Master Policy is terminated;
- the date your coverage lapses due to non-payment of premiums;
- the first of the month following your written request to terminate
this insurance;
- March 1 coincident with or next following the date you turn
age 65;
- March 1 coincident with or next following the date you retire;
- the date you cease to be covered by a Provincial Health Insurance
Plan;
- the date you are no longer a resident in Canada;
- March 1 coincident with or next following the date your membership
in the Canadian Professional Sales Association terminates;
which ever is earliest.
Coverage under this plan for your spouse will end on:
- the date your insurance terminates;
- March 1 coincident with or next following the date your spouse
turns age 65; or
- the date of your divorce or separation;
which ever is earliest.
Coverage under this plan for your dependent children will end
on:
- the date your insurance terminates;
- the date your children reach the maximum insurance age;
- the date your children are no longer dependent upon you for
support and maintenance; or
- the date your children marry;
which ever is earliest.
Exclusions and Limitations
Your CPSA Health Care insurance covers reasonable and customary
eligible expenses incurred in Canada, which result from an accidental
injury or sickness. Certain drugs and expenses are not covered
under this plan.
CPSA - Dental Care (rider to Health Care Plan)
The CPSA Dental Care insurance covers routine dental services.
Certain expenses and procedures are not covered under this plan.
If you elect to purchase Dental Care insurance with your Health
Care Plan you must pay premiums for the Dental insurance for one
full year.
Overview
Good oral health is an important element of your overall well-being.
And since your provincial plan covers little if any dental care,
our Dental coverage may be just right for you.
Routine Care under both options is covered at 80%. This includes
exams, x-rays, cleaning, fillings and extractions. Endodontics
and Periodontics (gum treatment, root canals) are covered at 50%
under the Basic Plan. Endodontics are covered at 80% and Periodontics
(gum treatment, root canals) are covered at 60% under the Deluxe
Plan. There is a $25 single/$50 family deductible under the Basic
Plan but no deductible under the Deluxe Plan. The Annual Benefit
Maximum (for all dental services combined) is $1,000 per person
(the Endontic/Periodontic maximum is $500 per year).
Note: The Dental Care Plan is available if purchased
with the Health Care Plan. For example, if you have purchased
the Basic Health Care Plan you can purchase the Basic Dental Care
Plan or if you have purchased the Deluxe Health Care Plan you
can purchase the Deluxe Dental Care Plan.
