Overview
Our CPSA Accidental Death and Dismemberment Plan protects you
and your family (if you elect the Family Plan) in the event of
an accident anywhere around the world, 24 hours a day, 365 days
a year - even if they are work related. It pays a cash benefit
if you die or are severely injured as a result of an accident.
Eligibility
This plan is available to all CPSA members in good standing under
age 65* (working full time) and can be extended to your spouse
(under age 65) and unmarried dependent children under age 22 (25
if in school full-time and dependent on you for support). "Child"
means your natural or legally adopted child, or step-child.
Acceptance to this plan is guaranteed since there are no medical
exams or questionnaires required.
Note: CPSA members age 65 to 85 and their spouses age
65 to 85 may each purchase $15,000 of insurance under this plan.
(However, this insurance does not cover "loss-of-use",
education benefit, or the following benefits described under "other
features": rehabilitation, occupational retraining, child
day care, or waiver of premium.).
Amount of Insurance
If you're under age 65, you can purchase coverage from $50,000,
up to $750,000 - no medical exam required. If you purchase the
Family Plan, your spouse and dependents will be insured for a
percentage of the principal sum you select for yourself. Members
age 65 to 85 and their spouses age 65 to 85 can purchase $15,000
of protection.
What's Covered?
This plan pays a benefit in the event of an accidental injury
or death. The benefit amount paid is a percentage of the principal
sum (the amount of coverage purchased) and is based on the severity
of the loss. The plan also includes a "loss-of-use"
provision which pays a benefit if there has been a permanent and
total loss of use for twelve continuous months.

Notes: If an accident causes more than one loss, only
one percentage (the greatest amount) would be payable.
Your coverage will increase by 10% in 2nd year of participation
and 25% in 3rd year, with no increase in cost to you.*

Education Benefit
If you have selected the Family Plan and have $150,000 or more
coverage, each of your children would receive an education benefit
of up to $5,000 a year for up to four consecutive years if your
death is accidental and occurs while the child is attending (or
is within one year of attending) a recognized post-secondary institute.
Repatriation Benefit
If the insured person dies as a result of an accident outside
of Canada, the plan will pay the expense (up to $10,000) of returning
their body home for burial.

It also contains a Seat Belt benefit (which increases the benefit
paid if the injury or death is caused by a car accident and the
insured person is wearing a properly fastened seat belt); and
a premium waiver benefit which pays the premium for this plan
on your behalf in the event you become disabled for six consecutive
months before age 65 and are unable to work. The plan covers accidents
while flying as an aircraft passenger (so long as the aircraft
is certified airworthy).

The Family Option
The Family Plan insures your spouse and dependent children for
a percentage of the principal sum you select for yourself.
Exclusions
This plan does not cover loss caused by suicide, sickness, disease
or infection, intentionally self-inflicted injury, war or act
of war, or service in any country's military, naval or air service.
Termination of Coverage
Coverage under this plan will end on:
- the date the Master Policy is terminated;
- the date your coverage lapses due to non-payment of premiums;
- the first renewal date (March 1) following attainment of age
70; or
- the first renewal (March 1) following the date your CPSA membership
terminates
whichever is earliest.
Coverage for your spouse and/or dependent children will end on
the date your coverage terminates or the date they cease to be
eligible; which ever is earliest. For members and their spouses
age 65 to 85 who have purchased the $15,000 accident benefit,
coverage ends on March 1 coincident with or next following the
attainment of age 85.
