The plan will cover:
The plan will pay up to (per insured person):
- Hospital room (in Canada) Semi-private/private room (paid
at 100%)
- Convalescent hospital room (in Canada) Semi-private room -
max 180 days
- Out-of-Canada emergency care
- hospital room 90 days of hospitalization per occurrence
- physician/surgeon fees Reasonable and customary fees
- Prescription drugs/medicines, including drug card* The cost
of generic drugs - if no generic exists, the plan pays for brand
name drugs
- Ambulance/air ambulance (emergency transport to and from nearest
hospital with adequate facilities)
- Emergency dental care due to an accident $750 per tooth -
treatment must be within 104 weeks of accident
- Private duty nurse/nursing assistant** $10,000 per injury
or sickness per 12 months
- Orthotics*** $200 for both shoes and inserts per calendar
year
- Health practitioners:** $500 per practitioner per calendar
year Acupuncturist, chiropodist, chiropractor, massage therapist,
naturopath, osteopath, podiatrist, psychologist, physiotherapist,
speech therapist (plus required x-rays/lab tests)
- Hearing aids (not batteries) $500 every five years
- Blood/plasma; rental (or purchase if less expensive) of medical
equipment/supplies e.g., artificial limbs or eyes, casts, splints,
trusses, crutches/braces, oxygen/ equipment, wheelchair, hospital
bed
- Diagnostic lab and x-ray expenses
- Repatriation to home province/territory in Canada $15,000
- Provincial/territorial user fees (where permitted by law)
This plan covers eligible expenses over and above those paid
by your provincial or territorial government health plan, subject
to certain limits and subject to an annual deductible. The deductible
is $50 per insured person per calendar year, to a maximum of $100
per family per calendar year.
* MaritimeScript "pay-direct" drug card allows Maritime
Life to pay the pharmacy directly. You only have to pay the portion
of the cost not covered by the plan.
When you cover your dependent children, you must advise Maritime
Life, in writing, of any child born after the effective date of
dependents' coverage. Coverage is effective as of the child's
birth so long as Maritime Life has been advised and has received
the appropriate premium (if any) within 45 days of the birth.
Otherwise, coverage is effective as of the date Maritime Life
is advised and approves the application. Out of province and/or
Canada coverage is limited to the first 60 consecutive days per
trip. Where more than one trip occurs within a six month period,
coverage is limited to the first 75 days of travel, in total.
Certain exclusions and limitations apply, including a pre-existing
condition limitation.
Travel assistance
Membership in the Health Care plan also entitles you to a travel
assistance program. This program is designed to provide on-the-spot
help in case of a medical, legal or personal emergency while traveling
outside your home province. Members can access help by calling
the travel assistance provider 24-hours a day. Information will
be sent to you outlining how the program works, how claims are
handled, as well as a description of the benefits available.
Conditions
The Health Care plan covers eligible expenses over and above those
paid by your provincial or territorial government health plan.
1. Overall maximum: Benefits are subject to an overall
(lifetime) maximum limit for each insured person. That amount
is:
To age 65: $1,000,000 of paid expenses in and out of Canada
combined
Age 65 and over: $50,000 of paid expenses in Canada, plus
$12,500* incurred out of Canada *NOTE: Since $12,500 would not
provide adequate protection in the event of a major medical emergency
outside Canada, we recommend that you consider purchasing additional
travel insurance if you are not covered elsewhere.
Reinstatement of overall maximum: On the 1st of January
of each calendar year, each insured person's overall coverage
is automatically reinstated by $5,000, regardless of age (so long
as the reinstatement does not exceed the overall maximum limits
above).
2. The plan does not cover expenses paid or payable by any Worker's
Compensation law, any other government program or any other insurance
plan.
3. User fees charged under provincial or territorial Medicare
plans are covered where permitted by law.
4. Maritime Life is entitled to be reimbursed for any expenses
paid which you recover (or are entitled to recover) from a third
party.
5. The plan covers eligible expenses incurred as the result of
injury or sickness but does not cover expenses for cosmetic treatment
or surgery except when necessitated by an accident.
6. The plan does not cover expenses resulting from self-inflicted
injuries; war or act of war; service in the armed forces; travel
for health; periodic health examinations; or health examinations
required by a third party.
